Use the following steps to access and fill in the web profile form:
1. Depending upon how your system administrator has configured the system, you will receive either an email containing a link to the Web Profiling form (if you have a valid email address in the Actionable Findings system) or a fax that contains a link that you must type into your browser, which allows you to access the Web Profiling form. See the illustrations below for examples of each method (email on the top, fax on the bottom).
2. Once you click (or enter) the link, a login window opens, requesting the PIN number that was included in your email or fax.
3. Enter your PIN (and Profile ID if not automatically entered) and click Submit. The Web Profiling form opens.
Any of your information that previously existed in the system is automatically populated in the form. In the illustration, below, an email address is already provided.
4. Enter any Account Information and Office Information that you want to include in your profile.
5. In the Device Information section, enter the requested fields for a Primary, Backup, and/or FailSafe device.
Primary: Device that the initial notification will be received upon.
Backup: An additional device or recipient to receive the notification on at the 2nd escalation point.
FailSafe: An Additional device to receive the 3rd and final notification.
NOTE: You can use the same device for all three stages if you choose to do so. |
NOTE: You can add an SMS provider that is not included in the SMS drop-down list. Just select any of the carriers from the drop-down list, then enter the correct carrier name in the Additional Comments or Notes section. |
6. In the Additional Comment or Notes for your Account section, enter any additional comments that you would like to provide to the administrator about your devices or demographic information.
7. Select Advance Notification Routing items as desired. These check boxes indicate to your administrator whether you want to specify a workflow based on:
Findings levels or categories: Allows you to request more than one notification criticality level; perhaps a cellphone for Primary notifications and email for other notification levels. Or if you have an email address and a smart phone, enter your email address and request information on the downloadable Mobile Clinician Application (see step 7 on the next page).
Schedules: Select this check box if you would like to have an administrator apply one or more schedules to your profile. For example, you might want to be notified at a specific cell number (or email address) on Monday, Wednesday, and Friday between noon and 8:00 PM, and an alternate number on Tuesday and Thursday between 7:00 AM and 7:00 PM.
Clinical Teams: Select this check box if you are part of a clinical team of two or more doctors. Your administrator will then contact you for more details on these requests.
8. Click the link in the Mobile Clinician Phone App area to view instructions for downloading and installing the Mobile Clinician phone application.
9. When finished click Submit. The information is updated in your Actionable Findings profile for your institution.
